Ugly Bathroom Contest
Grand Prize: Full bathroom makeover from Tile Art of Fort Collins, Colorado Total value of the grand prize is up to $8,500 (see value details below).

WHO CAN ENTER
1. Entrants must be legal residents in Larimer or Weld counties of Colorado, who are at least 21 years of age or older and own their home.
2. Employees of Tile Art and/or Fix My Shower, its members, contest sponsors, affiliated companies, advertising and promotion agencies, and the immediate family members (i.e., spouses, parents, grandparents, children, grandchildren, and siblings) of each of the above are NOT eligible to enter and/or to win. This contest is subject to all applicable federal, state, and local laws and regulations. Void outside the United States and where prohibited.
ENTRY
1. Beginning July 1st, 2010 through July 31st, 2010 person’s age 21 and older who own a home may qualify to enter the Ugly Bathroom Contest by submitting up to three photos to FixMyShower.com. Simply fill out your name and email in the box on the right of the website and you will receive an email with a url and upload instructions for your photos. Just click on the url in the email and follow the instructions on the page. Entries may also be mailed to Fix My Shower, 530 Nokomis Ct. Fort Collins, CO 80524. Up to 3 pictures of your bathroom can be submitted.
2. Pictures must be submitted or postmarked by midnight on July 31st, 2010.
3. All qualifiers must be homeowners.
4. Entries may be submitted on a third party’s behalf provided the third party is 21 years of age or older and owns the home in which the entered bathroom is located.
5. No purchase is necessary to enter or win. A purchase will not increase your chance of winning.
6. Tile Art and Fix My Shower and their sponsors are not responsible for any entries that are misdirected, lost, not received or are illegible.
7. Tile Art and Fix My Shower Management shall be the sole arbitrator in all matters relating to the contest and the interpretation of contest rules. The decisions of Tile Art and Fix My Shower Management shall be final. Entry into this contest constitutes agreement by contestant to abide by these rules, as well as any other specific contest rules established by Tile Art and Fix My Shower. Entrants are required to provide truthful information and Tile Art and Fix My Shower will reject and delete any entry that it discovers to be false or fraudulent. Tile Art and Fix My Shower will disqualify any entry from individuals who do not meet the eligibility requirements, and Tile Art and Fix My Shower will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
PRIZE(S)
1. On line voting for one of the entries will be July 15th, 2010 through August 15th, 2010. The entries with the largest vote total will win the grand prize. Only one vote per person will be permitted.
2. One grand prize will be awarded on August 23rd, 2010 and will include a bath remodel from Tile Art of Fort Collins, Colorado with a value of up to $8,500, which may include a bath or tub remodel, flooring, fixtures, grab bars, shower seats, or shower doors.
3. Prizes may not be substituted for cash.
4. Eligible bathrooms shall be with-in our regular operating area of Larimer and Weld Counties
5. Products will take up to 6 weeks to arrive and then the installation will be scheduled by Tile Art.
6. Extensive repair to the existing shower wall is not included
7. All provisions & restrictions contained in Tile Art and Fix My Shower and their business partner’s/sponsor’s standard contracts shall apply
8. Tile Art and Fix My Shower Management reserves all rights to interpret these rules and their decision(s) shall be final
9. Winners are required to present a valid photo ID in order to redeem any prize. Prizes are not transferable and may not be given, bartered, sold, or exchanged for another prize.
10. Payments of all federal, state and local taxes are solely the responsibility of the winner. Winner will be required to complete and submit an IRS Form W-9 or the equivalent including the winner’s full Social Security Number for receipt of any prize valued at $600 or more. Failure to submit a complete W-9 or equivalent will result in disqualification and forfeiture of the prize.
11. Prizes are not redeemable for cash. No substitution, transfer or assignment of prizes is allowed.
12. By participating in this contest, each contestant hereby consents to the use of contestant’s name, voice, likeness, biographical information and his/her participation in the contest for advertising with Tile Art and/or Fix My Shower and promotional purposes. Contestant agrees that no additional compensation will be paid for such usage.
ODDS OF WINNING/MISCELLANEOUS
1. (1) Grand Prize: Bathroom Makeover. Limit 1 Grand Prize available. The odds are based on the number of entries into the “Ugly Bathroom” promotion. Prizes are not transferable and may not be given, bartered, sold, or exchanged for another prize.
2. Winners also may not grant interviews about their participation in any Tile Art and/or Fix My Shower contests or make public comments in any medium, including the Internet without prior written permission of Tile Art and/or Fix My Shower. Additionally, no prize may be used in any way that would be disparaging to Tile Art and/or Fix My Shower.
3. Tile Art and Fix My Shower Management reserves the right to change, alter or modify these rules at any time with or without written notice.
4. Tile Art and Fix My Shower, at their sole discretion, reserves the right to disqualify any person (and all of their entries) from this contest if he or she, directly or indirectly, tampers with the entry process, the operation of the contest, voting or the operation of Tile Art and/or Fix My Shower web channels or is otherwise in violation of the rules. Tile Art and Fix My Shower further reserves the right, at their sole discretion, to modify, cancel, terminate or suspend the contest, or any part of it, if it is not capable of completion as planned or if any fraud, technical failures or any factor beyond the promoter’s control, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort, corrupt or affect the administration, security, fairness, integrity or proper conduct of the contest as determined by Tile Art and Fix My Shower at their sole discretion. Any attempt by an entrant or any person to deliberately damage any Tile Art and/or Fix My Shower web site or to undermine the legitimate operation of this contest may be a violation of criminal and civil laws. Should such an attempt be made, Tile Art and Fix My Shower reserves the right to seek full prosecution and/or damages from any such individual to the fullest extent permitted by law. Tile Art or Fix My Shower’s failure to enforce any term of these Official Rules shall not constitute a waiver of this provision. If due to circumstances beyond the control of the Sponsors, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Sponsors reserve the right, but not the obligation, to modify, suspend or cancel the contest and shall not be required to award a substitute prize.
5.The Sponsors are not responsible for (i) typographical or other errors in the printing, the offering or the administration of the contest, or in the announcement of a prize; (ii) entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties, malfunctions, disconnections, other technological failures, telephone service outages, delays, dropped calls, or busy signals; or (iii) lost, stolen, mangled, misdelivered, postage due, illegible, incomplete, incorrect, or late entries.
6. If the number of entrants into the contest does not meet or exceed ten (10) Tile Art and Fix My Shower management reserves the right to either extend contest deadlines until the above criteria are met or cancel the contest at their sole discretion.
